Terms and Conditions for all promotions

Individual promotions cannot be used in conjunction with any other promotion and are not substitutable or redeemable for cash.
To redeem the promotion, the patient must mention the promotion when booking the appointment.
Services will only be provided if suitable and appropriate.
Additional treatment may be recommended at an additional cost. Any cost or treatment will be discussed with you before being carried out.
Promotions subject to appointment availability.
Prices are subject to change.
The practice does not intend to in any way or form to encourage patients to take their treatment decisions based on financial gain. The offer should be solely considered a token of the practice's gratitude.
It is the patient’s responsibility to enquire about the cost of treatment with the dentist beforehand.

Offer commences from 9am AEST Wednesday 5th June 2024 and ends 5.30pm AEST Wednesday 31st July 2024.
Appointments must go ahead and be completed by Wednesday, 31st July 2024 in order to qualify for the promotion.
* Dental examination, scale/clean and fluoride treatment all to be taken in one single appointment / booking to receive $40 total treatment fee.
- For existing patients: dental examination, scale/clean and fluoride treatment all to be taken in one single appointment
- For new patients: dental examination, scale/clean and fluoride treatment all to be made in one booking, but dental examination will be one appointment and scale/clean and fluoride will be a in second appointment.
Fee adjustments will be applied on completion of treatment.
Offer is open to both new and existing patients.
Promotion only applies to patients seeing Dr Jordan Hawkins, Dr Kah Chong, and Dr Xiao Jin.
Treatment includes periodic exam, scale and clean, fluoride treatment, oral cancer screen and routine x-ray.
For private health fund members $40 discount is applied on the total treatment fee, and the respective health fund contribution will be applied to the new discounted total treatment fee.

Referral payments to be paid in increments (first 50% at successful employment of referral, second 25% on referral successful passing six month probation period, final 25% payment on referral reaching one year employment) direct to referrer’s nominated bank account in their name.
Referral payments figures of $500 (for Dental Assistant) and $1000 (for Oral Health Therapist) are inclusive of Australian Goods and Service Tax (GST).
Referral payments are only valid for open job positions as advertised by Snowy Mountains Dental.
There is no limit to the number of people you refer or the number of bonuses you can receive.

Snowy Mountains Dental has no control over your eligibility for Afterpay or the amount they allow you to use. This is between you and Afterpay.
When you choose to use Afterpay as a method of payment at our practice, you agree to their terms and conditions which are found on their website: https://www.afterpay.com/en-AU/terms-of-service

Snowy Mountains Dental has no control over what your private health insurance pays towards your dental treatment.
We can process your health fund card on our HICAPS machine as a complimentary service. When we do this, they will pay any benefits to us directly on your behalf to save you having to claim back the benefits yourself after paying us in full. However, what your health fund pays towards your dental treatment is entirely between you and your health fund.
It is your responsibility to contact your health fund regarding their terms and conditions, especially the benefits they provide and limits and restrictions on these.
Snowy Mountains Dental does not accept responsibility when your health fund does not pay the benefits that you think they should.
You are responsible for paying your account whether your health fund contributes to it or not.
